Heart Failure Nurse/LPN

The CHARM (Community Heart Failure Assessment, Rehabilitation and Management) Clinic at Advanced Cardiology is Alberta’s ONLY community-based outpatient clinic that runs on a charitable basis with the support and donations from the DIL WALK Foundation and Advanced Cardiology. The clinic is physician-directed, but patient care is managed by a nurse.

We are looking to hire an RN/LPN in Calgary for a Maternity leave Contract, as a Heart Failure Nurse. Previous experience in cardiology is an asset but not necessary. Training will be provided.

Job Summary:

• The Heart Function Nurse is responsible for performing assessments, developing a plan of care and implementing and evaluating the care.
• Knowledge of Heart Failure necessary to obtain and interpret information in terms of the patient’s needs.
• Knowledge and skills necessary to modify are according to patient’s needs and ability.
• Knowledge of nursing and pharmacy procedures and standards of practice, both general and specific, to the specialties of heart failure.
• Responsible for direct patient care of chronic heart failure patients in CHF.
• Assesses educational needs of patients with a diagnosis of heart failure on an as needed basis.
• Responsible for heart failure education.
• Ability to work in collaboration with Cardiologist, Heart Failure Specialist, Internal Medicine and Endocrinologist.
• Demonstrate leadership ability. Ability to prioritize and organize. Ability to interact effectively with diverse individuals at all levels of authority.
